Job Description:
To provide services to meet the social and/or emotional needs that affect the residents' ability to achieve their highest level of function; participate in the development of residents' comprehensive care plans; develop policies and procedures to provide social services to residents in compliance with federal, state and local regulations.
Functions:
- Develop and implement social service policies and procedures to meet residents' needs in compliance with federal, state and local regulations.
- Develop, coordinate and participate in family and resident activities designed to promote social interaction, reality orientation and intellectual stimulation.
- Develop one-to-one professional relationships with residents and families as needed for counseling in dealing with death and dying and other emotional, mental environmental or physical limitations.
- Assess and complete the sections of the (MDS) Minimum Data Set assigned completely, timely, and accurately. Document the social service component of the Comprehensive Care Plan for each resident in a timely manner.
- Document psycho-social needs, interaction and follow-up action(s) with resident and/or family according to facility policies and procedures.
- Assess, plan and document residents' discharge needs in accordance with facility discharge planning policy and regulatory requirements.
- Act as a liaison with social, health and community agencies. Refer residents to social, health and community agencies and complete accurate documentation in residents' records concerning the results of such referrals.
- Take part in facility in-service programs as required.
- Organize community events related to resident's needs and interests. Plan, schedule and implement room visits for residents unable to leave their rooms.
- Participate in admission of residents as assigned.
- Coordinate and cooperate with the activity department to plan and conduct events to meet socialization needs of the residents.
- Accept assigned duties in a cooperative manner. Perform other related duties as directed by his/her supervisor. Perform all duties assigned in an effective, timely and professional manner.
- Participate in implementation of programs to improve resident care related to Quality Indicators. Assume Quality Assurance duties as assigned.
- Assume accountability for all data contained in the employees' handbook.
- Come to work as scheduled and consistently demonstrate dependability and punctuality.
- Come to work in neat, clean attire and consistently present an appropriate professional appearance.
- Consistently work cooperatively with residents, residents' representatives, facility staff, physicians, consultants and ancillary service providers.
- Transport residents to doctor visits and other appointments as required.

Qualifications:
- A college graduate in a related field of study approved by the licensing agency; or complete a training course approved by the State.
- Must have some computer knowledge (Internet Explorer, Microsoft Office, Microsoft Windows XP)
- Must be able to be on feet for extended periods of time
- Must be able to lift, stoop, bend, squat, etc. without restrictions

Education:
- Minimum: High school diploma or GED
- Preferred: At least two years of college from an accredited institution.
|
Certification:
- Must have current CMA license
|
Continuing Education:
- As required by state and facility, including seminars.
|
Schedule:
- Monday - Friday
