Job Description:

Responsible for all direct care staff on the assigned shift. He/She is to provide direction, supervision, and evaluation of resident care. Implements facility policies, procedures and coordinates nursing activities with other services provided by members of the health care team.

  • Follows established standards of nursing practice and implements facility policies and procedures on the assigned unit.
  • Interprets existing policies and procedures to nursing assistants, restorative nursing assistants, residents, families and physicians.
  • Supervises and evaluates all direct resident care provided and initiates corrective action as necessary.
  • Obtains report from nurse he/she is relieving and records sufficient information for follow up action as necessary.
  • Provides report to nurse coming on duty, including sufficient information for follow up action as necessary.
  • Conducts resident rounds daily, reports problems to nursing supervisor and initiates corrective action promptly.
  • Identifies safety hazards and initiates corrective action immediately.
  • Identifies resident problems and emergency situations and initiates immediate lifesaving measures in the absence of a physician.
  • Supervises and evaluates implementation of the "Resident Rights."
  • Participates in facility education programs and coordinates student activities with the director of nursing education.
  • Provides clinical supervision of nursing assistants providing direct resident care.
  • Attends staff meetings, in-service classes and committee meetings as assigned.
  • Follows staffing guidelines and makes nursing assistant's assignments according to the qualifications of staff and characteristics of the resident population.
  • Uses supplies and equipment efficiently and effectively.
  • Assumes responsibility for compliance with federal, state, and local regulations within the assigned unit.
  • Counsels nursing assistants and recommends disciplinary action, when necessary, to the Director of Nursing.
  • Participates in the employee evaluation process by providing information on the clinical performance of nursing assistants to the Director of Nursing.
  • Administers medications and treatments per the physician's order and accurately records all care provided.
  • Accurately, completely, and timely documents resident care provided and residents' response or lack of response to care provided.
  • Reports changes in residents' conditions to a physician, director of nursing, and responsible party promptly and takes follow-up action as necessary.
  • Follows facility admission procedures and communicates resident admission to other departments, physicians, and ancillary service providers.
  • Follows facility discharge/transfer procedures and coordinates discharge/transfer with other departments, physicians and ancillary service providers. Completes accurate, timely documentation related to the resident's discharge/transfer.
  • Demonstrates concern for all residents' call for assistance.
  • Controls angry feelings appropriately.
  • Demonstrates sensitivity to feelings of others by listening and taking follow up action as necessary.
  • Supervises prompt response to residents' call for assistance.
  • Greets residents by name and speaks to residents and family members, demonstrating a kind, thoughtful manner.
  • Listens to resident, family, or physician concerns attentively and initiates corrective action and/or reports concerns to nursing supervisor promptly.
  • Works where assigned with a cooperative spirit.
  • Comes to work as scheduled, on time, and consistently demonstrates dependability and punctuality.
  • Comes to work in a neat, clean uniform and consistently present a professional appearance.
  • Assumes personal responsibility for obtaining continuing education related to long-term care nursing.
  • Current State License registration
  • Graduate of an accredited school of nursing.
  • Issued by the State
    Continuing Education:
  • As required by state and facility, including seminars.
