Title Activity Director

Job Description:

Plan, coordinate, and direct resident activity programs utilizing resources of the facility and community. Conduct activities in accordance with physician orders. Participate in the development and implementation of the assessment process and resident care plans and maintain necessary required records and documentation.

  • Plan, schedule and implement a program of individual and group activities based on residents' schedule.
  • Use the resident assessment protocols (RAPs) to determine whether to proceed to care planning for concerns related to a resident's activity program needs.
  • Gather data and document required ancillary assessments including but not limited to resident assessment protocol summary documentation.
  • Identify and document in the resident's care plan, the activity interests, needs, and concerns of the resident. Care plans are reviewed and updated as necessary but at least quarterly.
  • Document all interaction with resident and/or family in the assessment, care plan and progress notes as required by federal and state requirements.
  • Participate as an active member of the interdisciplinary team and attend all resident care conferences. Plan and implement Reality Orientation programs when appropriate. Participate in nursing restorative and rehabilitative programs.
  • Plan and conduct Residents' Council meetings. Plan and implement evening and weekend functions as necessary.
  • Take part in in-service education programs and attend all in-service sessions relating to Activity Program.
  • Organize and schedule community events related to residents' interests.
  • Supervise barber and beauty parlor activities.
  • Plan, schedule and implement room visits and in-room activities for residents unable to leave their rooms.
  • Plan, schedule and implement indoor and outdoor activity programs.
  • Maintain an activity attendance record for each resident.
  • Assess and maintain an adequate supply of materials to implement activity programs.
  • Observe infection control procedures related to the Activity Program, including identification of residents not appropriate to attend out-of-room activities.
  • Accept assigned duties in a cooperative manner.
  • Assume accountability for data contained in the employee's handbook.
  • Follow all facility policies and procedures.
  • Assume Quality Assurance duties as assigned.
  • Come to work in neat, clean attire and consistently present an appropriate professional appearance.
  • Come to work as scheduled and consistently demonstrate dependability and punctuality.
  • Consistently work cooperatively with residents, residents' representatives, facility staff, physicians and ancillary service providers.
  • Must have certificate from the State approved program or degree that qualifies
  • Must pass OSBI background check
  • Must be able to be on feet for extended periods of time
  • Must be able to squat, bend, stoop, etc without restrictions
  • Must be able to lift without restrictions
  • Minimum: High school diploma or GED
  • Preferred: At least two years of college from an accredited institution
  • Issued by the State
    Continuing Education:
  • As required by state and facility, including seminars.
